To be considered for this requirement, you must respond with the attached SF1449; no other format will be accepted. Fill out the following: Blocks 17a and 30a-c on page 1. Cursive font signatures will not be accepted. Vendor information on page 3 Input pricing with CLINs starting on page 4 to include unit price, amount, and net amount. Also be sure to fill in the blank for "per item per week" price within the CLIN description. Look at the Performance Work Statement on page 53-54 for further details of requirement.
