Defense Priorities and Allocations Systems (DPAS) rating is DO-C9. A Section 508 does not apply. The Government’s preferred method of payment is credit card. If the offeror does not accept Mastercard or a surcharge is applicable, payment may be made through Wide Area Workflow (WAWF). Award will be made to the offeror whose quote represents the lowest priced technically acceptable (LPTA) offer. In order to be deemed technically acceptable, an offeror must provide the following: 1. Assertion that the offeror proposes to perform this tasking utilizing board certified psychiatrist(s) or licensed clinical psychologist(s). 2. Provide copy of current licenses and certifications. 3. Provide three (3) past performance examples describing experience performing tasking similar to this requirement Statement of Work. The past performance examples must be within three (3) years of the closing date of this solicitation. 4. Provide address of location where the offeror will perform the evaluations. The address must be within sixty (60) miles of NUWCDIVNPT (1176 Howell St., Newport, RI 02841), as calculated by a standard Google Maps search. 5. The Offeror must state any potential Conflicts of Interest 6. The Offeror must provide pricing for all base and option CLINs. 7. The Offeror must complete, sign, and submit Attachment 2 – FAR 52.204-24 with its quote. Additionally, the Government will consider past performance information in accordance with DFARS Provision 252.213-7000, where negative information within SPRS may render a quote being deemed technically unacceptable.
