We would like to request a quote for a service contract to rent and launder 207 laboratory coats per week with a Period of Performance of: Base Year - 18 April 2019 through 17 April 2020 Option Year One - 18 April 2020 through 17 April 2021 Option Year Two - 18 April 2021 through 17 April 2022 Option Year Three - 18 April 2022 through 17 April 2023 Option Year Four - 18 April 2023 through 17 April 2024 Please indicate Company Name, Full Address, Point of Contact Name, phone number and E-Mail address on your quote. Quote must also include the company Commercial and Government Entity (CAGE) Code. Contract will be for a base year plus four option years. Contract will be written that includes funding for the Base Year and the dollar values but no funding for the four Option Years. As the first Option Year approaches, both NHRC and Vendor will have the option to exercise or chose to not exercise the Option Years. If both NHRC and vendor agree, at the end of the Base Year, funding will be added to the first Option Year. The same would be applicable to the second through fourth Option Years. Quote to include breakdown of total weekly charges with a calculation of what the total yearly cost of the contract will be. A quote is required for the base and each option year. Laboratory Cleaning Schedule: •· Must pick up soiled coats and drop off clean coats at least 1 time every week. •· Delivery day during the week must be on the same day every week. •· This schedule shall vary by one day if there is a Federal holiday or site closure during the week. Pick-up and drop-off on Wednesdays is preferred. •· Delivery time must be between 8:00 AM - 11:30 AM on the selected day. •· Must contact NHRC if there are any delays in schedule. •· The standard term of service shall be no more than 7 days (including weekends) between pick-ups/drop-offs. Rental laboratory coats must have the following features: •· Unisex fit, in all available sizes (i.e., extra small, small, medium, large, extra large) (quantity of each size to be determined) •· Material: cotton/polyester blends •· Snap buttons in the front, running up to the neck for full coverage •· Long coat length •· Long sleeves with fitted cuffs •· Coat must have at least 1 pocket •· 3 different color choices must be available for use. White must be one of the mandatory colors. Quantity •· Minimum of 3 lab coats, in 3 different colors, for each laboratory staff member (9 total per staff member). (18 laboratory staff and 5 visiting scientists - 23 * 9 = 207 weekly) Replacements •· Must provide appropriate replacement services for coats that become heavily soiled, torn, or otherwise unwearable Address: General delivery address is: Naval Health Research Center McClelland Road and Patterson Road Gate 4, Building 330 San Diego, CA 92152 This contract will require base access for all personnel delivering laboratory coats. In order to access Naval Health Research Center at Naval Base Point Loma, service providers will need to be enrolled in the Defense Biometric Identification System (DBIDS). Providing registration information is voluntary. However, failure to provide Social Security Number and Driver's License Number or other acceptable identification may result in a denial of access.
