Make sure there is a Point of Contact Name, Email, and Phone Number on your submittal. 2. RETURN - RFQ (all pages), Vendor Information Sheet - filled out completely. See the attachments for the RFQ and any other documentation. Ordering Data Sheet and Statement of Work are imbedded in the RFQ.
