All quotes shall include completed copy of the solicitation with the prices listed in the pricing table and the required solicitation provisions and clauses certified and filled as needed, together with the a completed copy of the Reference Sheet. Also required is the Past Performance Questionnaire, which shall be emailed directly to the government by the offeror's references.
