All quotes must be submitted only to the Contracting Officer, Kaitlyn Garcia, kaitlyn.garcia@nexweb.org and the Contract Specialist, Lea Delacruz, leanat.delacruz@nexweb.org, in accordance with the RFQ. Please complete the following before returning your quote: 1. Complete Blocks 11, 13, 14, 15 and 16 on the top page of the RFQ 2. Complete Attachemnt 4 (Price/Cost Breakdown) 3. Complete Attachment 5 (Contractor Past Performance) 4. Complete Attachment 6 (Section K, Provisions) Any and all RFIs shall be submitted to both the Contract Specialist and Contracting Officer to the emails provided above.
