All interested vendors, for your bid to be considered submit a quote of your product with descriptions and or item part numbers for evaluation but also fill out and submit section A of this RFQ. Filling out section B of this solicitation and submitting it with only pricing of the items does not suffice as a bid for evaluation purposes. Failure to comply will disqualify your bid, as the information in section A along with a viable quote is required, along with an up to date SAM registration. All interested vendors who submit a bid are responsible for following directions by filling out and submitting the necessary articles being asked in this posting for your bid to be considered. It is not the responsibility of the Contract Specialist to follow up with what the vendor is missing upon submission. All interested vendors upon submission of an offer, invoices of at least 2 are required from either private or public institutions of these products. Refer to section B, C, and H for product information and salient characteristics of this RFQ for details concerning base access and requirements needed to fulfill solicitation. Only offers for ALL requirements will be considered. Answer: Yes, the items listed beneath the description are what make up the kit. Question: We're looking for a SOW on exactly what needs to be done. Please advise. Is it just to deliver these items? Is it a one time opportunity or how long is this contract opportunity? Answer: This is a one-time opportunity for the delivery of the items requested. Question: Do these products need to be delivered all at once or within a 30 period? Answer: The end user agrees to delivery within a 30 day period. Question: I was wondering what kind of brushes were you meaning exactly? Is it cleaning brushes for teeth? or overall cleaning brushes? Answer: cleaning brushes for the piezosurgery heads…. Question: I am also wondering was there a particular brand request for these cleaning items: · Enzymatic cleaning solution · Barrier milk · Cleaning brushes Also, the quantity for these cleaning items says one. Are they only wanting one pack of cleaning brushes? Since they vary in sizes and diameter, was there a particular size they wanted for the instruments? Do they want a variety of brushes? You placed a unit number for these items, and the brushes are a little strenuous to find, I am wondering if they want the brushes purchased in a pack of a certain amount? or one pack of a certain number of brushes inside? Does the brand of the brushes, barrier milk and cleaning solution make a difference? How many gallons or what size should the liquid solutions be? (milk barrier, cleaning solution) Answer: The end user did not specify a definitive quantity, size or product type.
