REQUEST FOR INFORMATION (RFI) Obstetrics Patient Safety Conference Coordinator INTRODUCTION: This Request for Information (RFI) is issued solely for information and planning purposes. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. This RFI does not commit the Government to contract for any supply or service. Your response to this RFI will be treated as information only and will not be used as a proposal. No entitlement to payment, of direct or indirect costs, or charges to the Government will arise as a result of a contractor's submission of a response to this announcement or the Government's use of such information. The information provided may be used by the Government in developing its acquisition strategy, statement of work/statement of objectives, and performance specifications. Interested parties are responsible for marking proprietary or competition sensitive information contained within their response. Not responding to the RFI does not preclude participation in any future solicitations. The information provided in this RFI is subject to change and is not binding on the Government. PURPOSE: 502 CONS / JBKBA is conducting market research to gather information on the availability of qualified sources to provide for an Obstetrics Safety Conference Coordinator. The contractor shall provide an Obstetrics Patient Safety Conference for education and training purposes, IAW the attached “Draft” Performance Work Statement (PWS). The Government anticipates the use of North American Industry Classification System (NAICS) Code 611430, Professional and Management Development Training, with a small business size standard of $12 million, for this requirement. DESCRIPTION: The contractor shall provide an Obstetrics Patient Safety Conference for education and training purposes. As part of their duties, the contractor will be responsible for preparing, organizing, and conducting a two-day Critical Care/Safety Educational Conference for San Antonio Military Medical Center (SAMMC) obstetric providers and nurses that is open to all Military Health System providers. See Draft PWS for detailed technical requirements. RESPONSE FORMAT: As part of your capability statement, provide responses for each of the following questions, utilizing the existing numbering scheme for each response and limiting your total response for this RFI to no more than three (3) pages: 1. Business Information: A. Company Name, CAGE code and DUNS code. B. Mailing Address. C. Point of Contact (to include e-mail, phone/fax). D. Web site, if applicable. E. Socioeconomic/Business Size Status for NAICS code 611430. Please provide your business set-aside status (i.e. 8(a) Business Development Program, Small Business, Small Disadvantaged Business (SDB), Historically Underutilized Business Zone (HUBZone), Woman-Owned Small Business (WOSB), Service Disabled Veteran-Owned Small Business (SDVOSB), Large Business, etc.). Please identify all that apply. 2. State your experience in providing obstetric patient safety conferences. 3. Give examples of existing or past contracts where you provided obstetric patient safety conferences. 4. Describe your capability to meet the technical description IAW the draft Performance Work Statement. ADMINISTRATIVE INFORMATION: Small Business (SB) Set-Aside Determination. The Government reserves the right to decide whether or not a SB set-aside is appropriate based on responses to this notice. In order for the Government to make a SB set-aside determination, it is emphasized that SB concerns provide sufficient written information to indicate their capability to successfully perform and manage all the requirements of this magnitude.
