Mill & overlay the majority of Gott Rd. from Young Rd. to Kline Ave. 2. Full depth asphalt replacement of critical areas 3. Full depth asphalt patching 4. Curb & gutter repairs CONTRACTOR REQUIREMENTS: The Contractor awarded will need to provide information to have business set up for payment. Also will need to fill out forms for Reps & Certs, W-9, & Authorization Agreement For Vendor Automatic Electronic Funds Transfer. If you are large business you will need to be set up on 45 days net terms and small business will be 30 days Net terms. You must also provide three References of similar like work within the past two years, and Technical Approach in your bid package. Pre-Bid to look at the site is scheduled on 19 August 2020 at 10:00 a.m. Central Time zone.
