This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This Request for Quotes (RFQ) is being issued in accordance with FAR Part 13.303 Blanket Purchase Agreements. This RFQ incorporates all provisions and clauses in effect through the Federal Acquisition Circular 2021-04. The solicitation is issued as HU000121R0005 on Contract Opportunities. It will be the responsibility of the Contractor to monitor Contract Opportunities for any issuance of any amendments to the solicitation. This competition is open to all businesses (full and open competition). The North American Industry Classification System Code (NAICS) for this requirement is 621511, Medical Laboratories with a size standard of $35.0 million. Offerors interested in responding to this requirement must provide a quote list for the Blood Sample Analysis Tests listed in Attachment 1 SOW. USU has a requirement to establish a Five (5) Year Blanket Purchase Agreement (BPA) for Blood Sample Analysis Testing. This includes the blood sample analysis tests listed in Attachment 1 SOW. The BPA Call Limit is $10,000 and the BPA Ceiling is $250,000. Please see attached RFQ and Attachments for full detail.
