NOTICE TO CONTRACTOR This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services, in conjunction with FAR Part 13 Simplified Acquisition Procedures, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a separate written solicitation will not be issued. The solicitation number is HT941025Q2047. It is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2025-03 effective 01/17/2025 and Defense Federal Acquisition Regulation Supplement (DFARS) effective 1/17/2025. The North American Industry Classification System (NAICS) Code for this acquisition is 541380. The size standard is $16.5 (Millions of Dollars). This solicitation is 100% set aside for small businesses. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. This requirement is for: Biosafety Cabinet Maintenance and Certification for the Defense Health Networks Pacific Rim, Central, and Indo-Pacific as listed in Attachment 1 (Statement of Work (SOW)). The Government intends to award a five (5) year (and six (6) month extension) firm-fixed-price, single-award, non-personal service Blanket Purchase Agreement (BPA). Defense Health Agency (DHA) Healthcare Contracting Division – West (HCD-W) requests responses from qualified sources capable of providing the following: The contractor shall provide a trained Field Certifier(s) who shall perform field tests and certification for the listed (attachments 4 and 5) equipment items in the Defense Health Networks (DHN) Pacific Rim, Central, and Indo-Pacific. The contractor will provide labor, all supplies, and transportation for technicians to perform the services per the Original Equipment Manufacturer (OEM) specifications, Department of Defense Manual (DoDM) 6055.18 Safety Standards for Microbiological and Biomedical Laboratories, local command instructions, and the Joint Commission Hospital Accreditation Program requirements in accordance with (IAW) the SOW.
