Notice of Intent to Sole Source HT941023N0111 THIS IS A NOTICE OF INTENT TO AWARD A SOLE SOURCE NON-PERSONAL SERVICES CONTRACT AND IS NOT A REQUEST FOR COMPETITIVE QUOTES. The Defense Health Agency Contracting Activity (DHACA), Western Markets Contracting Division (WMCD), San Diego, intends to negotiate and award a firm fixed price contract for various Original Equipment Manufacturer (OEM) test reagents and laboratory supplies in accordance with Federal Acquisition Regulation FAR 13.106-1(b)(1)(i), only one source being available to meet the Government’s needs, to: Henry Schein, Inc. 135 Duryea Road Melville, New York, 11747-3824 The North American Industry Classification System (NAICS) code for this requirement is 325413. The business size standard is (1,250 employees). The Product Service Code (PSC) is 6550 The objective of this sole source award is to procure non-personal services for United States Naval Hospital, Okinawa Japan. The anticipated period of performance is for base with four (4) option years. Ten (10) OEM brand name test reagents and laboratory supplies (see table below) are required to conducts critical clinical testing for military patients and their beneficiaries IAW the shipping schedule and unique requirements described within the contract on an annual basis. These reagents are used in conjunction with the existing analyzers to test and diagnose said patients and beneficiaries for various medical conditions and diseases and to ensure compliance with the College of American Pathologist (CAP) procedures (see the attached delivery schedule and product spreadsheet). These reagents are temperature sensitive and have wide variations of shelf-life stability. Reagents shall have a minimum of 6-months of shelf life remaining upon receipt, and enough shelf life remaining to overlap the next shipment of the same items to ensure the materials do not expire prior to use at the Military Treatment Facilities (MTFs). Items shall arrive within the required manufacturer temperature range. The required reagents and equipment shall be drop shipped directly by the contractor to OCONUS medical facilities in Japan. The items shall integrate the utilization of the uniquely configured existing installed equipment and manual bench testing. No other products will be accepted due to the compatibility with OEM testing platforms to reduce the high risk or loss of life due to insufficient testing, inaccurate diagnosis, and improper shipping procedures. Henry Schein is an Original Equipment Manufacturer (OEM) authorized dealer, authorized distributor, and authorized reseller for the required supplies. This notice of intent is not a request for competitive proposals, and no solicitation document exists for this requirement. Sources interested in responding to this notice are required to submit a capability statement that includes management and technical data and cost information, in sufficient detail and with convincing evidence that clearly demonstrates the capability to provide the required services for Naval Hospital Twentynine Palms. Capability statements shall not exceed six (8.5 x 11 inch) pages using a font size no smaller than 10-point. All capability statements received by the due date of this notice will be considered by the Government. A request for documentation or additional information or submissions that only ask questions will not be considered as an affirmative response. A determination by the Government not to compete based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement or to proceed with a sole source purchase order.
