See attached document titled, "HT940822Q0012 AMENDMENT 1." ------------------------------------------------------------------------------------------------------------------------------------------------------------------- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is HT9408-22-Q-0012. The solicitation is issued as a Request for Quotation (RFQ). Provisions and clauses in effect through the Federal Acquisition Circular (FAC) 2022-06 are incorporated. It is the Quoter’s responsibility to be familiar with the applicable provisions and clauses. The North American Industry Classification System (NAICS) code is 334510, Electromedical and Electrotheraputic Apparatus Manufacturing, with a small business size of 1,250 (size standards in number of employees). The requirement will be fulfilled as a 100% small business set-aside. The Defense Health Agency Contracting Activity (DHACA) Southern Market Contracting Division (SMCD) is issuing this RFQ on behalf of the Uniformed Services of the Health Services (USUHS) School of Medicine Department. This requirement is for three (3) Brand Name or equal Fujifilm Sonosite Edge II Ultrasound Systems with supporting parts, materials and warranty.
