Amendment 0002 to this RFQ HT940625QE008 is to: (1) modify the Performance Work Statement (PWS), Section 2.3.3.3; (2) change the Delivery Schedule for Line Items 0001 through 0004; (3) address the question received in response to RFQ HT940625QE008. Amendment 0001 to this RFQ HT940625QE008 is to provide the drawing to Room 1072 at Branch Health Clinic (BHC) Key West, Florida. This acquisition is for the Naval Hospital Jacksonville (NHJAX), Florida, in support of the Branch Health Clinic (BHC) Key West, Florida. The contractor provide all personnel, equipment, supplies, transportation, tools, materials, supervision, and other items and non-personal services necessary to disassemble and remove the existing audiometric test booth. The contractor shall supply and install one (1) new two-person audiometric test booth for conducting hearing screenings in support of the BHC Key West. The contractor must custom fit and assemble the new audiometric test booth in the location identified in the Performance Work Statement (PWS). The contractor shall disconnect, reconnect, and reconfigure the audio and electrical devices to the new audiometric test booth in accordance with (IAW) the original equipment manufacturer's (OEM) specifications, the newest United States, state and local law, Defense Health Agency (DHA) regulations, and The Joint Commission (TJC) requirements. The contractor must provide final testing and calibration certification and verification of operability after completing the installation and reconnection of the new audiometric test booth at BHC Key West, Florida.
