Synopsis/Solicitation HT940624R0008 is issued as a Request for proposal (RFP). The synopsis/solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Regulation as indicated in the attached Standard Form (SF) 1449. The SF1449 form is being utilized to generate the applicable clauses from the Standard Procurement System. The Defense Health Agency Contracting Activity, Southeastern Healthcare Operations Contracting Division intends to procure a firm fixed price contract for Steris equipment maintenance services in support of Naval Hospital Guantanamo Bay. The on-site annual preventative maintenance, calibrations, and repairs services for the Steris equipment must be performed by original equipment manufacturer certified personnel at Guantanamo Bay, Cuba. Contractors must be able to gain base access withing 60 days after contract award. The contract will consist of a base year and four Option years of 12 months each. This requirement is being solicited in accordance with Federal Acquisition Regulation Part 13 – Simplified Acquisition Procedures. Competition Approach: Full and Open competition. Submission of Proposals: Contractors must follow the requirements of FAR 52.212-1 Addendum, 52.212-2 Addendum, and the Proposal Instructions and Evaluation Criterion provided in the solicitation document to create the proposal. Proposal is required to be submitted electronically to the Contracting Officer and Contract Specialists on or before the date and time specified in SF 1449, Block 8 of Page l. Period of Performance: The estimated start date is March 1, 2024. The period of performance shall be for one (1) Base year of 12 months and four (4) Option years of 12-month each. The start date is subject to change. The Defense Health Agency Contracting Activity, Southeastern Markets Contracting Division intends to procure a firm fix price contract in support of US Naval Hospital Guantanamo Bay’s (USNHGB) Sterilizing Processing Department (SPD) and Dental Department. The contract will consist of a base Period of Performance (POP) of 12 months and four Option periods of 12 months each. The POP start date is February 01, 2024. Scope: The contractor shall perform on-site preventive maintenance, preventive maintenance required parts replacement and installation, calibration, corrective maintenance (repairs), and field service reports for STERIS washer disinfector and sterilizer equipment, AMSCO® Century® 26x37.5 Medium Steam Sterilizers, InnoWave Unity Ultrasonic Irrigator, Reliance Genfore Washer Disinfector, AMSCO 5052, Innowave Ultra+, and V-PRO 60 Low Temperature Sterilization System. When there are product recalls, the contractor shall process and install equivalent for each equipment and must coordinate with USNHGB’s Biomed Medical Repair Division point of contract before installation of recalled equipment or parts. The services provided per equipment shall be in accordance with the Original Equipment Manufacturer (OEM) specifications and the Joint Commission Hospital Accreditation Program requirements. The contractor shall provide OEM Certified Field Engineers and Technicians who have gone through OEM training to perform services on the STERIS equipment. The contractor shall provide training certificates prior to the performance start date of any service(s) under this requirement. Training certificates and/or certifications shall detail training location, trainee’s name, training completion date, and training expiration date. Place of Performance: The services must be provided at the US Naval Hospital Guantanamo Bay, Cuba. To be considered for award, firms must be registered in the System for Award Management (SAM).
