To be considered responsive to this solicitation you must complete the following on the attached SF1149: Respond to blocks 17a and 30a-c on page 1. Complete all the vendor information on page 3. Provide your prices (both unit price and net amt) on the CLIN beginning on page 4. No other format except for the SF 1449 will be accepted and cursive font signatures aren't acceptable.
