All interested vendors, for your bid to be considered submit a quote of your product lease with descriptions, and or item part numbers, for evaluation but also fill out and submit the Solicitation Information of this RFQ. Filling out Solicitation Information of this solicitation and submitting it with only pricing of the items does not suffice as a bid for evaluation purposes. Failure to comply will disqualify your bid, as the information in Section A along with a viable quote is required, along with an up-to-date SAMs registration. All interested vendors who submit a bid are responsible for following directions by filling out and submitting the necessary articles being asked in this posting for your bid to be considered. It is not the responsibility of the Contract Specialist to follow up with what the vendor is missing upon submission. All interested vendors, note that upon submission of an offer at least 2 invoices from a non-government facility are required showing price of the lease of these products. Refer to the Performance Work Statement (PWS) for product information, Period of Performance, and salient characteristics of this RFQ for details concerning base access and requirements needed to fulfill solicitation. Only offers for ALL requirements will be considered. All responses must be received by September 02, 2023 at 3:00PM eastern standard time. ***This is an all or nothing solicitation, brand name or equal based on salient characteristics.*** Questions concerning this request for Quotation shall be submitted, via email to Jessica.M.Collins40.civ@health.mil before August 30, 2023 at 3:00PM EST, to allow sufficient time to respond.
