To be considered for this opporutunity, the vendor must complete and return the attached SF1449; no other format will be accepted. To complete the SF1449, fill out blocks 17a and 30a-c on page 1, all of page 3, and insert vendor's prices beginning on page 4 within the CLINs. For pricing, the vendor must insert the unit price as well as the net amount.
