Please respond with the attached SF1449; no other format will be accepted. Please respond to blocks 17a and 30a-c on page 1 and the vendor information on page 3, as well as provide your price(s) on the CLIN(s) beginning on page 4. Cursive font signatures will not be accepted. ***Please read and adhere to the attached RFQ.*** All interested vendors, for your bid to be considered submit a quote of your supplies with descriptions and or item part numbers for evaluation and fill out and submit Section A of this RFQ. Filling out section C of this solicitation and submitting it with only pricing of the items does not suffice as a bid for evaluation purposes. Failure to comply will disqualify your bid, as the information in section A along with a viable quote is required, along with an up-to-date SAMs registration. All interested vendors who submit a bid are responsible for following directions by filling out and submitting the necessary articles being asked in this posting for your bid to be considered. It is not the responsibility of the Contract Specialist to follow up with what the vendor is missing upon submission. All interested vendors upon submission of an offer: submit at least 2 invoices from either private or public institutions of these supplies. Refer to section C for product information and salient characteristics of this RFQ for details concerning requirements needed to fulfill solicitation. Only offers for ALL requirements will be considered. All responses must be received by Saturday, May 6th by 5:00 p.m. Eastern Standard Time (EST) Questions concerning this request for Quotation shall be submitted, via email to Sheila.L.Pusey.civ@health.mil before 12:00 p.m., EST, 3rd May 2023 to allow sufficient time to respond. No questions will be accepted after 12:00 p.m., EST, 3rd May 2023.
