Filling out section B of this solicitation and submitting it with only pricing of the items does not suffice as a bid for evaluation purposes. Failure to comply will disqualify your bid, as the information in section A along with a viable quote is required, along with an up-to-date SAMs registration. All interested vendors who submit a bid are responsible for following directions by filling out and submitting the necessary articles being asked in this posting for your bid to be considered. It is not the responsibility of the Contract Specialist to follow up with what the vendor is missing upon submission. All interested vendors upon submission of an offer, invoices of at least 2 are required from either private or public institutions of these services. Refer to section B, C, and H for product information and salient characteristics of this RFQ for details concerning base access and requirements needed to fulfill solicitation. Stryker Sales, LLC is the manufacturer of several Stryker Endoscopy equipment types and the sole provider for service, OEM parts, technical support, and contractual maintenance for each. A service contract through the original manufacturer ensures proper functioning of the equipment. Stryker Sales, LLC does not train, nor authorize any other organization within North America to service or maintain the Stryker Endoscopy Towers. Only Stryker Sales, LLC can provide maintenance service to meet the minimum requirements of the Government. Only offers for ALL requirements will be considered. All responses must be received by Wednesday, April 12th by 10:00 a.m. Eastern Standard Time (EST) Questions concerning this request for Quotation shall be submitted, via email to Jessica.M.Collins40.civ@health.mil before 10:00 a.m., EST, 11th April 2023 to allow sufficient time to respond. No questions will be accepted after 10:00 a.m., EST, 11th April 2023.
