Request for Quotes GENERAL INFORMATION Product Service Code: Q301Medical- Laboratory Testing NAICS: 621511 Medical Laboratories Place of Performance: 52d Medical Group’s (MDG) Laboratory at the Medical Treatment Facility (MTF) Spangdahlem Air Base, Germany. The Defense Health Agency Contracting Activity (DHACA) Northeastern Markets Contracting Division (NM-CD) on behalf of the 52d Medical Group’s (MDG), is issue a solicitation for a Blanket Purchase Agreement(BPA) for non-personal services as described in the attached Statement of Work (SOW). The Contract is anticipated to be awarded as a Firm Fixed Price (FFP). The anticipated period of performance is 12 months from September 30,2023 through September 29, 2024, plus four option years. REQUIREMENT DESCRIPTION: Non-Personal Services 1. The Contractor shall provide qualified personnel, services, materials, equipment, supplies and facilities necessary to perform follow-up newborn metabolic screening as a non-personal service in support of the 52th Medical Group’s (MDG) Laboratory at the Medical Treatment Facility (MTF) Spangdahlem Air Base, Germany, in accordance with the Statement of Work (SOW), (Attachment #1). 2. Please complete the Requirements List (Attachment #2) according to paragraph 2 and paragraph 3 of the Statement of Work and return it with your quote/price list. 3. All quotes must be submitted in the English language to be considered for contract award. 4. All vendors must have a DUNS number, a CAGE code, and shall be registered in WAWF to be considered for issuance of an agreement. 5. The government will award a Blank Purchase Agreement (BPA) resulting from this solicitation to the offeror whose offer conforming to the Solicitation will be most advantageous to the government. The government will evaluate quotes based on price, technical acceptability, and conformity to the solicitation.
