The Defense Health Agency seeks information on capabilities of commercially available Root Cause Analysis (RCA) methods or tools. In 2003, the agency first procured a license to a methodology for RCA and currently uses these services. The agency is interested in methods and tools that could support the agency's patient safety program without undertaking a significant software configuration effort. The agency is additionally interested in accompanying services that could facilitate transition from legacy to new tools and methods.
