This is a combined synopsis and solicitation for Medical Paper Shredding for one base year plus four (4) option years. See attached Performance Work Statement (PWS), dated 21 July 2020 with Clauses and attached Pricing Worksheet. The Offeror shall provide 31 secure bins at prescribed locations to accommodate an estimate of 4,500 LBS of material to be picked up twice per month and shredded onsite per the PWS and must be able to perform all services in full. This requirement is solicited on a competitive unrestricted basis for North American Industrial Classification Standard (NAICS) 561990. CLIN Structure : 0001 The contractor shall provide all materials, labor, tools and equipment necessary to accomplish shredding services for 31 document bins twice per month for 12 months as stated in the Performance Work Statement (PWS) Dates and Place of Delivery: The requested service is for a 1-year base and 4-1 year option periods and shall take place for the following periods: base year; 21 Sept 2020 – 20 Sept 2021 option 1; 21 Sept 2021 – 20 Sept 2022 option 2; 21 Sept 2022 – 20 Sept 2023 option 3; 21 Sept 2023 – 20 Sept 2024 option 4; 21 Sept 2024 – 20 Sept 2025 *option; 21 Sept 2025 – Up to 6-months *option to extend services per FAR 52.217-8 Hill Air Force Base 75 MDS/SGSM Hill AFB, Utah 84056 Quote as follows for each POP listed above on the attached pricing sheet and return with your proposal. Prices must be submitted exactly as shown on the attached price sheet for all performance periods. This synopsis and solicitation is prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and therefore quotes are being requested. This solicitation is issued as a request for quote (RFQ) FA820120RMedShred. Submit only written offers; oral offers will not be accepted. All firms or individuals responding must be registered with the System for Award Management (SAM) at www.sam.gov. Offerors will be evaluated on a Lowest Price Technically Accepted (LPTA) and the most advantageous to the Government, considering price, technical capability, and past performance, in accordance with FAR Parts 12 and 13. The Offeror shall provide a technical capability statement showing their ability to perform these services at Hill AFB Utah. The Offeror shall provide a minimum of three (3) past performance references of similar size and scope which may include any government agency. Provide name, contact name and number, contract number and period of performance. To determine technical acceptability, the Government will review the technical proposal and provided past performance to determine if the Offeror is capable to perform the work as described in the PWS. If it is determined to be in the best interest of the Government, we may conduct interchanges with an Offeror or Offerors. The Government may consider the correction potential of proposals in determining the content and extent of interchanges. The purpose of the interchange may be to correct errors, address deficiencies or gain an understanding of aspects of a proposal. The Government may conduct interchanges either orally or in writing, and may issue interchanges to one, some, or all Offerors. The Government intends to award a contract to the lowest priced technically acceptable Offeror. The Government reserves the right to award on a multiple award or an all or none basis.
