This solicitation has been cancelled effective 8/26/2020 Pathology Services Hill AF Base FA8201-20-R-Pathology2 This is a combined synopsis and solicitation for Medical Pathology Consultant Services for one base year plus four (4) option years. See attached full Performance Work Statement (PWS) and Pricing Worksheet. The Offeror must be able to perform all services provided in the PWS. Please quote a fully burdened firm fixed on the Pricing Worksheet (attached with this solicitation). Offers will be evaluated on a Lowest Priced Technically Acceptable basis (LPTA) with a documented and acceptable Past Performance Rating of Satisfactory or better in CPARS under the Q515 PSC. Item 1: Pathology Consultation Services Dates and Place of Delivery: The requested service shall take place between base year; 1 Dec 2020 – 30 Nov 2021 option 1; 1 Dec 2021 – 30 Nov 2022 option 2; 1 Dec 2022 – 30 Nov 2023 option 3; 1 Dec 2023 – 30 Nov 2024 option 4; 1 Dec 2024 – 30 Nov 2025. option 5; 1 Dec 2025 – up to 6-months IAW FAR 52.217-8 Location: Hill Air Force Base, UT This synopsis and solicitation is prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and therefore quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quote (RFQ) FA8201-20-R-Pathology2. Submit only written offers; oral offers will not be accepted. All firms or individuals responding must be registered with SAMS at https://www.sam.gov This procurement is a Small Business SET-ASIDE for North American Industrial Classification Standard (NAICS) 621111. Funds are not presently available for this contract action. The Government’s obligation under this contract action is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until you as the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer, in accordance with FAR 52.232-18.
