This is a combined synopsis and solicitation for Medical Pathology Consultant Services for one base year plus four (4) option years. See attached full Performance Work Statement (PWS) and Pricing Worksheet. The Offeror must be able to perform all services provided in the PWS. Please quote a fully burdened firm fixed on the Pricing Worksheet (attached in this solicitation). If you have any questions/comments regarding the requirement, please submit via e-mail to below by 8:00 AM Mountain Time, 20 March 2020. Note: timeliness of services is non-negotiable. Offers will be evaluated on a Lowest Priced Technically Acceptable basis (LPTA) with a documented and acceptable Past Performance Rating of Satisfactory or better in CPARS under the Q515 PSC. Item 1: Pathology Consultation Services Dates and Place of Delivery: The requested service shall take place between base year; 1 June 2020 – 31 May 2021 option 1; 1 June 2021 – 31 May 2022 option 2; 1 June 2022 – 31 May 2023 option 3; 1 June 2023 – 31 May 2024 option 4; 1 June 2024 – 31 May 2025. option 5; 1 June 2025 – up to 6-months IAW FAR 52.217-8 Location: Hill Air Force Base, UT This synopsis and solicitation is prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and therefore quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quote (RFQ) FA8201-20-R-Pathology. Submit only written offers; oral offers will not be accepted. All firms or individuals responding must be registered with SAMS at https://www.sam.gov This procurement is a Small Business SET-ASIDE for North American Industrial Classification Standard (NAICS) 621111.
