This is a combined synopsis and solicitation for the purchase of - four (4) Eckel AB 4240 Mini Audiometric (or equal) Booths. Please provide a quote which includes the (4) Booths meeting or exceeding the minimum specifications of a Eckel AB 4240 Mini Audiometric Booths including all shipping and installation costs. Also the quote shall include the noise/check and certification of each booth. Booth size required – Outside dimensions of 42”x 40”x75” with Inside dimensions 38’x36”x68”. The booths must be on casters so that they can be moved. Booths must also be able to be uninstalled/installed by our staff and not required to be uninstalled/installed by manufacturer or to have to hire someone to uninstall/install in the event that they must be moved into an area where they will not fit thru doorways. If providing an (or equal) product, your quote must include any specification sheets and diagrams needed to adequately show the product meets the (or equal) booths. It is recommended that you highlight the specifications showing your product meets the specified product. Any product deemed not meeting the specifications of the Eckel AB 4240 Mini Audiometric booths will not be considered for award. Offers will be evaluated on a Lowest Priced Technically Acceptable basis (LPTA) with a documented and acceptable Past Performance Rating of Satisfactory or better in CPARS under the 6515 PSC. Quotes shall include: 1) Company Name, Address, Contact Information, Dunns and Cage Code on the Company quote document. 2) Estimated Delivery Date 3) Payment Terms 4) Product Quotation to include Part#, Description, Qty, Price and Total Price. Item 1: 4 (ea) Hearing Booths Item 2: 4 (ea) Shipping and Installation Item 3: 4 (ea) Noise/check and certification Dates and Place of Delivery: Shipped for delivery within 30 days after receiving order. Installation and Certification within 10 days after delivery. This announcement constitutes the only solicitation and therefore quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quote (RFQ) FA8201-20-R-HearingBooths. Submit only written offers; oral offers will not be accepted. All firms or individuals responding must be registered with SAMS at https://www.sam.gov This procurement is a Small Business SET-ASIDE for North American Industrial Classification Standard (NAICS) 334510.
