All repairs will be made on-site at the Oklahoma City - Air Logistics Complex (OC-ALC), Tinker AFB, OK. Please send the actual solicitation form completed along with any other documentation to support your company’s quote. Please make sure information stated is readable and legible. This is a Firm Fixed Price procurement, so please state the price on your quote as all inclusive (i.e. shipping, taxes, fees, etc.). When returning the solicitation form, please complete the following: Please complete the Vendor Information, Pricing Information, and Proposed Delivery Date on page 2. Please “wet” sign, print the name, title of signer, and date signed on page 2. Please insert unit price and total amount for each line item on Page 3. Please complete all information in the fill-in provisions on pages 29-47 if necessary. If you are not the OEM, please provide a written statement and supporting documentation to show that you have completed previous repairs to the same or similar Cyril Bath equipment along with your ability to perform the required repairs according to the OEM specifications for this requirement.
