The purpose of this combined synopsis is to establish a Firm-Fixed Price (FFP) Blanket Purchase Agreement (BPA) with a five-year ordering period to be fulfilled by a Small Business (SB). The required training is a lifestyle medicine training package that includes Lifestyle Medicine Professional certification pre-requisite training credentialed by the American Board of Lifestyle Medicine and the American College of Lifestyle Medicine, Lifestyle Medicine Professional certification exam registration, Lifestyle Medicine Conference registration, and a virtual support workshop, in accordance with the Statement of Work (SOW)
