Amendment: Update SCA Wage Determination to the latest version. The services required involve mandatory requirements for Non-Appropriated Fund (NAF) employees to receive a physical examination as a condition of employment. Physical appointments must be available during all operating hours of the medical facility to include Saturday and Sunday. The requested period of performance is a base period plus four option years. NONPERSONAL SERVICES: The attending physician will perform a physical examination and complete the Certificate of Medical exam form (Optional Form 178) provided by the USAF NAF HR office. Physical appointments must be available on a walk-in basis Monday – Friday 8 a.m. to 8p.m., and Saturday and Sunday 8 a.m. to 5 p.m. Examinations will be conducted at the contractor’s medical facility.
