This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is FA6606-22-R-0005 and is being issued as a request for proposals (RFP). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-05 and Defense Federal Acquisition Regulation Supplement 2022-05355, and Air Force Federal Acquisition Circular 2021-0726. (iv) There are no set-asides for this acquisition and it will be advertised under full and open unrestricted competition regulations IAW FAR 6.1. The associated NAICS code is 621111, Offices of Physicians (except Mental Health Specialists). (v) The following are the line item numbers, items, quantities, and units of measure: *Please utilize the attachment titled “01-Bid Schedule”* 0001 – FIREFIGHTER PHYSICALS, 75 EACH 0002 – FOLLOW-UP EXAMS, 1 JOB (Not a biddable item) (vi) Description of requirements: *See attachment titled “02-PWS for Firefighter Physicals” for full explanation of requirements* The government anticipates one single award Firm Fixed Price contract. This requirement stems from the need for Pre-employment, annual, termination, and fitness-for-duty examinations for current and potential Firefighters/EM/Bio employees at Westover ARB. These examinations should include a questionnaire that captures employee baseline information on medical status and history, an annual physical examination, weight and body composition, annual blood tests, urine laboratory tests, Pulmonary function testing (spirometry), Electrocardiograms (ECGs), sleep disturbance screening, cancer and cardiovascular disease risk reduction, hormone imbalance awareness consultation, behavioral health screening, and cancer screening (colon, prostate, lung, cervical, bladder, oral, thyroid, and skin). The contractor shall furnish all personnel, equipment, materials, and facilities necessary to perform examinations as required by NFPA Standard 1582, OPM standards, DOD and AF regulations. (vii) Estimated dates of service are 16May22 thru 15May23. Delivery and acceptance will occur at the examination location.
