Contracting Office Address Department of the Air Force, Air Force Installation Contracting Agency, 31st Contracting Squadron, – Aviano Air Base – Via Pordenone, Area E, Edif 600, 33081, Aviano PN, Italy. Description This is a combined synopsis/solicitation for commercial products or commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This solicitation will utilize Simplified Acquisition Procedures as authorized by FAR 13.5, Simplified Procedures for Certain Commercial Products and Commercial Services. This announcement constitutes the only solicitation; quotes are being requested and a separate written/1449 solicitation will not be issued. This solicitation is not to be construed as a commitment by the Government. All information is be submitted at no cost to the Government. This solicitation will be issued as a Request for Quotes (RFQ) under solicitation notice FA568224Q8072. This solicitation document and incorporated provisions and clauses are this in effect through the Federal Acquisition Circular (FAC) 2024-05, Defense Federal Acquisition Regulation Supplement Publication Notice (DPN) 08/15/2024, and the Department of the Air Force Acquisition Circular (DAFAC) 2024-1206. The applicable North American Industrial Classification System (NAICS) Code is 621340 - Offices of Physical, Occupational and Speech Therapists, and Audiologists. No set-aside will be assigned to this acquisition. Solicitation Details Quotes are due on or before 4:00 PM, Central European Time (CET), on Friday, 13 September 2024. Submissions shall be made via email to the Contract Specialist, A1C Vincent Gans at vincent.gans@us.af.mil and Contracting Officer, Ms. Biannica Barksdale-Glay at biannica.barksdale@us.af.mil. All submissions emails must reference solicitation number FA568224Q8072. Questions are due on or before 4:00PM, Central European Time (CET), on Thursday, 05 September 2024. All questions shall be submitted via email to the aforementioned points of contacts. Place of Performance The anticipated contract will provide services at Aviano Air Base, Italy. Anticipated Contract Type/Period of Performance The anticipated contract will be a Firm Fixed Price (FFP) Contract detailed in the Attachment 6. All Travel CLINs will use US Government Estimated Pricing. Period of Performance: The anticipated contract will consist of one (1) 12-month Base Period, four (4) 12-month Option Period, and one (1) 6-month Optional Extension. The resultant contract award will include FAR clause 52.217-8, Option to Extend Services for a period of performance not to exceed a six (6) month extension of services. The total period of performance shall not exceed five (5) years excluding the additional six (6) months if FAR 52.217-8, Option to Extend Services is exercised. Additional Requirements The contractor shall provide a quote that fully meets the requirements of this RFQ. The submission of electronic documents, via email, will be the only acceptable method of delivery. The contractor is required to submit any spreadsheets in “working” form to ensure that the evaluation of pricing is accomplished in an expedited manner. Moreover, all spreadsheets submitted shall include the working formulas used to develop the priced effort. In addition to the spreadsheets, the pricing submission is to be provided that includes the following information: i. The validity date for the proposal ii. A listing of all ground rules and assumptions iii. A listing of any scope limitations and/or qualification issues iv. A listing of any exceptions taken to the pricing requirements of this RFQ v. An explanation of the methodology used in developing the CLIN prices This RFQ is subject to the provisions, clauses, and contract terms outlined in Attachment 2- Addendum to FAR 52.212-1 Instruction to Offerors, Attachment 3 – Addendum to FAR 52.212-2 Evaluation; and Attachment 4 – Clauses and Provisions. Each submission for this requirement, at a minimum shall contain the information below as well as adhere to any instructions outlined in the FAR provision 52.212-1 addendum (attached): o CAGE Code: o UEI No: o Payment Terms o Date Quote Expires: The FAR Provision 52.212-1, Instruction to Offerors – Commercial Products and Commercial Services, applies to this acquisition. Addendum to 52.212-1 is contained in Attachment 2 of this solicitation. The FAR Provision 52.212-2, Evaluation – Commercial Products and Commercial Services, applies to this acquisition. Addendum to 52.212-2 is contained in Attachment 3 of this solicitation. Offerors must comply with the FAR Provision 52.212-3, Offeror Representations and Certifications – Commercial Products and Commercial Services and ALT I. This RFQ is not authorization to begin performance nor does this RFQ obligate the Government for any cost incurred by the offeror associated with developing a submission (quote/proposal). The Government reserves the right not to award in response to this RFQ. Prior to the commencement of any activities associated with performance of this requirement, the Government will issue a written directive or contractual document signed by the Contracting Officer with appropriate consideration established.
