This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is FA5613-22-R-0006 and the solicitation is issued as a request for proposal (RFP). This acquisition is conducted in accordance with FAR Part 12 and 13 procedures. The 700 CONS/PKA Flight has a requirement to contract for Utilization Management & Case Management (UMCM) Support Services (Non-Personal Services) for the 86th Medical Group (86 MDG) at Ramstein Air Base, Germany, for the period 15 July 2022 – 14 July 2023 with four 1-year options through 14 July 2027. Places of performance and acceptance are the 86th Medical Group’s (86 MDG) facilities at Ramstein Air Base, Germany. All offerors must propose FOB (free on board) destination. Offerors shall use Attachment 1, Pricing Schedule, which includes all contract line item numbers (CLINs), quantities and units of issue, to fill in all unit prices and total amount blocks. Attachment 2, Performance Work Statement (PWS), contains the description of the requirement for UMCM Support Services for the 86th Medical Group’s (86 MDG). Changes made from the PWS posted with the UMCM Sources Sought notice are highlighted yellow. All clauses and provisions applicable to this acquisition can be found in Attachment 3, Applicable Clauses and Provisions. For the submission of Past Performance Information, please use Attachment 4, Past Performance Questionnaire (PPQ) UMCM. Offers and PPQs are due 04 May 2022 (former date was 21 April 2022) not later than 16:00 hrs Central European Summer Time (CET). AMENDMENT 1: The due date for submission of proposals is being changed from 21 April 2022 to 04 May 2022; see Attachment COMBO UMCM - Amendment 1. This document is required to be signed by the contractor and included as part of the proposal package. AMENDMENT 2: Attachment 5 is added to answer questions received in response to the solicitation. Deadline for proposals are extended to 18 May 2022, 1600hrs CEST; see Attachment COMBO UMCM - Amendment 2.
