As a member of the Behavioral Health team, the Clinical Nurse – Behavioral Health Care Facilitator (BHCF) Clinical Nurse (CN) will provide mental health clinical case management, care coordination and disease management. This includes, but is not limited to the conditions of depression, anxiety disorders, tobacco cessation, weight management, Traumatic Brain Injury and Post Traumatic Stress Disorder. The BHCF CN will also monitor and facilitate adherence to treatments, provide education, coordinate care, and maintain data registries. Lastly, the BHCF CN will perform the full scope of clinical nursing in the execution of outpatient mental health care.
