RFQ was ammended on 10 September to: revise clauses and provisions in the RFQ, revise the Statement Of Work (SOW) where most changes have been highlighted, and revise Attachment 3 and Attachment 4 to no longer require certification. Please note that items were also removed from the SOW which couldn't be highlighted. This contract covers non-personal services for all management, labor, supervision, transportation, equipment and materials, etc., to provide complete medical housekeeping services for medical, dental, veterinary and other patient care facilities on Kadena Air Base. This program provides for the aseptic cleaning, protection, and beautification of all rooms and areas identified for coverage at a particular installation to maintain a level of asepsis commensurate with the use of the facility, rooms, and areas. This program also has provisions for management of linen service and all categories of Regulated Medical Waste (RMW) per the National Institute for Occupational Safety and Health (NIOSH) as identified in the Performance Work Statement. Please carefully review and consider the attached Request For Quotation along with each of the seven attachments to provide an acceptable quotation. There is expected to be a facility site visit for interested parties held on 14 August at Kadena Air Base. Please complete Attachment 7 - Request form for Site Visit and submit to the identified Point Of Contact as soon as possible. Questions are due by 17 August, at 12:00 PM. Please complete Attachment 2 - Question Form to submit questions. Past Performance will be evaluated.
