Please see the attached Q&A No. 02. Solicitation Amedment 00001: The purpose of this solicitation amendment is to provide responses to questions received as of July 5, 2022. Please see the attached Q&A No. 01. Solicitation Initial Posting: This is a combined synopsis//solicitation for physical therapy services at Misawa Air Base, Japan. The solicitation number is FA520522QB050 and the solicitation is issued as a Request for Quotation (RFQ). This acquisition is for a commercial service and is conducted under the authority of the Federal Acquisition Regulation (FAR) Part 13 - Simplified Acquisition Procdures and FAR Part 12 - Acquisition of Commercial Items. A Fixed-Price type of Purchase Order contract is contemplated for any resuling contract entered into as a result of this soliciation. See attached Performance-based Work Statement (PWS) and RFQ. Responses to this solicitation must include sufficient information to establish the interested parties' bona-fide capabilitites of providing the service. Offerors must submit all questions concerning this solicitation in writing to martin.almazan@us.af.mil Any responses to questions will be made in writing, without identifiation of the questioner, and will be included in an amendment to the solicitation. Even if provided in other form, only the question responses included in the amendment to the solicitation will govern performance of the contract. All responses must be received no later than the date specified in the RFQ. Late responses will not be accepted. All responses must refernce the subject solicitation number.
