This is a combined synopsis/solicitation for commercial service prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. ii. Solicitation FA4877-19-Q-0375 is issued as a Request for Quotation (RFQ). iii. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-03 effective 12 June 2019. The DFARS provisions and clauses are those in effect to DPN 20190628 effective 28 June 2019. The AFFARS provisions and clauses are those in effect to AFAC 2018-0525 effective 25 May 2018. The procurement is being issued as a 100% small business set-asdie. The Associated North American Industrial Classification Standard (NAICS) code for this request is 314110 and size standard is 1500 employees. iv. Davis Monthan Air Force Base intends to award a firm-fixed price contract for the following: CLIN 0001 - Carpet removal and installation in building 130- QTY: 1 - Unit of Measure: Bundle The contractor shall furnish all labor, parts, tools, materials, equipment, transportation, personnel, management, and supervision necessary to remove existing flooring and install new carpet tiles in the 755 Operations Support Squadron building, ground floor/1st floor, and 2nd floor, building 130. Please see attachment 1 "Performance Work Statement" for any specifications. The contractor must comply with attachment 2 "Carpet Ordering Guide March 2019" and attachment 3 "Wage Determination 2515-5474 Revision #8" v. Prior to commencing work, the contractor shall coordinate the date and time for set-up, for carpet removal and installation with building 130 Facility Managers, SSgt Jimmy Rossado at (520) 228-1899, and Maj Jeremy Moon at (520) 228-9581. vi. The government will place an order with the offeror whose quote meets lowest price and technical acceptability. Technical acceptability is defined and referred to within this solicitation document as the offeror's capability statement to meet the performance work statement of the service. vii. Important Dates/Times (All Times are Arizona Local Time) The government will be hosting a site-visit to inspect the system and its components of which will be replaced within this requirement. Answers will be provided by Monday 9 September 2019, 11:00 am. d. All quotes shall be submitted by Wednesday 11 September 2019, 11:00 am.
