This is a Design-Build project to demo, repair and restore interior and exterior elements damaged as resulting from Hurricane Michael. Associated areas of repair are identified within this scope of work. The project is comprised of one building on Tyndall AFB and the related disciplines of work as specific tasks in Scope of Work (Appendix A). The Design-Build Team will consist of a Construction and design team Designer of Record (DOR) that shall perform the services herein and as described in this Statement of Work (SOW). ***15 June 2021 - Site Visit Sign in Sheet Attached. ***21 June 2021 - Amendment to SOW "Attachment 2 - Appendix A updated 15 June 2021" The changes include: 1. Changed from 4 legged tower to 3 legged tower, this was a typo (paragraph 1.1.1.1.8) 2. Added note to place tower as close to existing tower as possible for user FAA certification by end user (paragraph 1.1.1.2.3) 3. Added note indicating swing gate needs to face existing access road (paragraph 1.5.2) 4. Added clarification indicating that only equipment shelter requires shallow foundation, tower requires deep foundations (paragraph 1.1.1.1.9) UPDATE: 28 June 2021 Update to include the following attachments -Attachment 12 XLWU218112 Q&As -Attachment 13 RFI 006 Tower Signs -Attachment 14 Combined DA-Repair Multiple Communication Towers Tyndall Attachment 13 and 14 are from a previous Tower project and the information is intended to be a guide for Tower 9352. Any reference or direction to repair additional towers is not applicable.
