Question: Will a sales tax exempt form be provided for this contract? Answer: The Arizona Department of Revenue stated Arizona Form 5000M is the applicable sales tax exempt form. The solicitation date has been extended to Monday, February 4, 2019, 4pm (MT) and the bid sheets have been revised to correct formula errors and remove columns that should not have been priced. The solicitation date has been extended to Wednesday, February 13, 2019, 4pm(MT). Any questions are due by Monday, February 11, 2019, Noon(MT). Set-Aside has changed from Service-Disabled Veteran-Owned Small Business (SDVOSB) to Small Business (SB).
