COMBINED SYNOPSIS/SOLICITATION: This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals/quotes are being requested and a written solicitation is attached. Solicitation FA460019RA003 incorporates provisions and clauses effective through Federal Acquisition Circular (FAC) 2019-01, dated 20 December 2018. This requirement is an unrestricted (Full and Open Competition) solicitation. North American Industry Classification System Code (NAICS) is 334516 - Analytical Laboratory Instrument Manufacturing, with a size standard of 1,000 employees. All qualified small businesses under this industry are encouraged to submit quotes. DESCRIPTION OF REQUIREMENT: Hematology Analyzer Lease and Reagents (includes equipment, supplies/reagents, and training) for Ehrling Bergquist Clinic located at Offutt Air Force Base, NE. The period of performance for this contract is estimated to be 1 March 2019 - 30 Sept 2019 plus four one-year option periods and the six month option to extend services, for a total performance period not to exceed six-one (61) months.
