AMENDMENT 001 This office received some questions regarding the subject solicitation, and the answers are being broadcast for equal access to information by all. Question 1: (translated and adapted) "Is it necessary to comply with the schedule stated in the Performance Work Statement, Paragraph 2.4.4. "OHCP Hours of Operation"? Answer 1: The stated schedule is the desired. If the vendor cannot comply with it, the vendor can propose an alternative, but the Government does not have to accept it Question 2: (translated and adapted) Is an audiogram report by a specialized physician required? Answer 2: Per the exam appointment list of this contract, all we are requesting is a audiogram which is reviewed by the Occupational health physician. If there are any abnormalities, that is when Occupational health provider refers the employee to ENT. Question 3: (translated and adapted) What is the format for the Quality Control Plan required by the Performance Work Statement, Paragraph 2.8.2, "OHCP Quality Control Plan" Answer 3: Attached is a questionnaire (Attachment 4 - PMP-QCP Questionnaire) that whose answers will suffice as the OHCP Quality Control Plan.
