Addendum 0001 - Updated Combo, Extension of Dates and Site Visit. 52.212-1 Instruction to Offerors - Commercial Items Addendum In order to be determined responsive, all offers must be completed on the below Price List, respond directly to the requirements of this solicitation, and send the required information to the identified POC's. If your quote is determined to be non-responsive, it will not be further considered for award. A complete quote consists of the following: a) Complete the price and extended price on the Price List provided. b) Complete the bottom portion of page four (4) of the Combined Synopsis/Solicitation document. c) Review the entire solicitation and fill in all required provisions/clauses. If the annual certification has been completed in the System for Award Management (SAM), FAR 52.212-3 does not need to be completed. d) A detailed plan outlining your company's approach to complete the install. e) Documentation to show a minimum of 3 year of experience installing fiber. 52.212-2 Evaluation - Commercial Products and Commercial Services Addendum Evaluation of offers will be completed following the instructions outlined in this addendum. The process of evaluation will be as follows: Lowest Price Technically Acceptable (LPTA). All responsive offers will be ranked based on their overall price. Starting with the two (2) lowest price offers, evaluations will be conducted to validate compliance with the Statement of Work utilizing the submitted detailed plan and experience documentation. Once two (2) offers have been determined technically acceptable, evaluations will be concluded. It is the intention of the Government to award one (1) purchase order in response to this solicitation. The one (1) offer with the lowest price technically acceptable (LPTA) will be selected in response to this solicitation. Engineer, Furnish, Install, and Test (EFI&T) new Fiber Optic Cable (FOC) 12 strands of Single Mode (SM) FOC and inner duct to building 1231 from building 1250. 12 SM fiber cable and inner duct will need to be installed through each manhole/duct system (MHDS) routed from Bldg. 1250 as well being spliced into each MH, if required. Total cable distance is approximately 1400 feet for the distance including maintenance loops and approximately 1250 feet in the existing manhole/duct route as depicted in the CVC figure (see attached IGE). Vendors shall perform all work IAW with the SOW. Vendors shall maintain a Cage Code in SAM.gov with their company name.
