Little Rock AFB has a requirment for the repair of storm drains IAW the attached SOW. The contractor shall provide all qualified personnel, tools, equipment, materials and services required to repair 2 storm drains between B380 and B370 on Little Rock Air Force Base, AR 72099. See Figure 1 for locations. The contractor shall accomplish all services below to meet the requirements of this Performance Work Statement (PWS). Work shall be in compliance with all Federal, State and local regulations, codes and license requirements for storm sewers. Work shall be accomplished within 120 days of award. Instructions to Offerors - In order to be determined responsive, all quotes must be complete on the SF 1442, and respond directly to the requirements of this solicitation. A complete quote consists of the following: a) Complete the unit price, extended price, and total price blocks along with all information required in the table requesting company information, including vendor signature. b) Fill in required provisions and clauses. FAR 52.204-24 must be completed. c) All vendors must have an active registration in SAM at time of quote submission. d) Site Visit: An organized site visit is scheduled for Thursday 2 September 2021 at 1:00 PM CST. Participants are to meet at the front gate of Little Rock AFB @ 12:30 PM to allow time to obtain escort passes and accountability. This will be the only site visit conducted. Reservations for the site visit will be submitted no later than 31 August by 3:00 PM CST. Due to COVID-19 installation rules ALL attendees must wear a mask if the 6 ft. rule cannot be met and shall be worn inside all buildings. Little Rock AFB is not an open base and you must have a valid ID , vehicle registration, and insurance to access the base. All questions will be due to the POCs listed in the table below by 3 September 2021 at 1:00 PM CST. All reponses are due by 10 September 2021 @1100 CST. AMENDMENT 1 The purpose of this amendment is an administration change to rename Attachment 1 Performance Work Statement to Attachment 1 Statement of Work. AMENDMENT 2 The purpose of this amendment is to extend the quotes due date from 24 August 2021 @1100 CST to 26 August 2021 @1100 CST. AMENDMENT 3 The purpose of this amendment is to extend the due date for this solicitation from 26 August 2021 @1100CST to 10 September 2021 @1100CST.
