The 19th Civil Engineering Squadron of Little Rock Air Force Base is seeking to acquire Elevator Maintenance for multiple elevators on base in accordance with the attached Performance Work Statement. This will be a base plus four option year contract and will not start until FY22. 52.212-1 Instructions to Offerors - Commercial Items Addendum In order to be determined responsive, all quotes must be completed on the SF 1449, respond directly to the requirements of this solicitation, and sent to the identified POC's and 19CONS.TEAM3.ORG@us.af.mil. If your quote is determined to be non-responsive, it will not be further considered for award. A complete quote consists of the following: a) Complete the unit price and extended price (under "Supplies or Services and Prices/Cost"), complete blocks 12 and 17a, and complete/sign blocks 30a, 30b, and 30c. b) Review ENTIRE solicitation and fill in all required provisions and clauses. If the annual certification has been completed in the System for Award Management (SAM), FAR 52.212-3 does not need to be completed. c) All vendors must have an active registration in SAM at time of quote submission. d) Vendors must submit documentation with their solicitation that includes: -At least 2 but no more than 5 references that are recent and relevant for verification (technical acceptability will be rated on a pass or fail basis for the required experience. Recent is defined as projects that have been completed in the last three years. Relevance is defined as having the same magnitude in work as the solicitation requires. - Each reference will include contract information, job time frame, the magnitude of the job, whom the job was completed for, and a POC that can be contacted. Site Visit: An organized site visit is scheduled for 8 July 2021 at 10:00 AM CST. This will be the only site visit conducted. Participants are to meet at the front gate visitor center of Little Rock AFB @ 0930 to allow time to obtain escort passes and accountability. Due to COVID-19 installation rules, masks are required to be worn by all individuals on the installation who are not FULLY vaccinated. "Fully Vaccinated" is defined as being at least 14 days past receiving the SECOND dose of the vaccine. Vendors MUST RSVP to the POCs listed no later than 6 July 2021 at 4:00 PM CST on who will be attending the site visits. Little Rock AFB is not an open base and you must have a valid ID to access the base. Questions: All questions are due to the identified POC's by 12 July 2021 at 10:00 AM CST.
