AMENDMENT 01: The purpose of this amendment is to: 1. Incorporate Attachment 7 Questions & Answers (these are questions asked by interested contractors). 2. Incorporate an updated Attachment 1 Performance Work Statement (Paragraph 4.4 has been updated). 3. Move the quote due date from 5 July 2021 at 10:00 AM CST to 7 July 2021 at 10:00 AM CST PLEASE EITHER FILL OUT BOTH THE ORIGINAL SOLICITATION (FA446021Q0021) AND THE AMENDED SOLICITATION (FA446021Q00210001) OR FILL OUT AND SIGN THE ORIGINAL SOLICITATION WITH A WRITTEN ACKNOWLEDGMENT THAT YOU AGREE TO THE AMENDED SOLICITATION (FA446021Q00210001). The 19th Force Support Squadron is seeking to acquire a Testing Specialist on Little Rock Air Force Base for FY22. All work will be in accordance with Attachment 1-Performance Work Statement. Please be sure to review FAR 52.212-1 Instructions to Offerors-Commercial Items Addendum below. Lastly, please review Attachment 6-Updated Mask Guidance for Vaccinated Persons, which speaks to the updated face covering guidance for Little Rock Air Force Base. 52.212-1 Instructions to Offerors - Commercial Items Addendum In order to be determined responsive, all quotes must be complete, self-sufficient, and respond directly to the requirements of this solicitation. A complete quote consists of the following: a) Complete the unit price and amount (under "Supplies or Services and Prices/Cost"), complete blocks 12 and 17a, and complete/sign blocks 30a, 30b, and 30c. b) Completion of FAR 52.204-24 is required. Complete all other provisions and clauses as required. c) All vendors must have an active registration in SAM at time of quote submission. All questions regarding the solicitation are due no later than 29 June 2021 @1:00 PM CST. Please submit all questions to the POCs listed below, as well as 19CONS.TEAM3.ORG@us.af.mil.
