David Grant Medical Center (DGMC) on Travis AFB, CA has a requirement for Hemodialysis Equipment Maintenance/Repair. The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform the on-site maintenance and repair for all hemodialysis machines, central reverse osmosis equipment, central water purification system, and portable reverse osmosis machines with their associated carts at David Grant Medical Center, as defined in this Performance Work Statement (PWS) except for those items specified as government furnished property and services. The contractor shall perform to the standards in this PWS. Period of Performance: Base Year and Two Option Years THIS IS A REQUEST FOR INFORMATION PURPOSES ONLY This request for information (RFI) is for planning purposes only and shall not be considered as a Request For Quote (RFQ) or a Request For Proposal (RFP), or promise to use as a future RFQ or RFP. This request for Information(RFI) is to determine the availability of businesses to include small businesss(including the following subsets, Small Diasaventaged Businesses, BUHZones Firms: Cetified 8(a), Service-Disabled Veteran-Owned Small Businesses and Women Owned Small Business) to provide the required products and /or services. This RFI does not commit the Government to contract for any supply or service. Further, the Air Force is not Currently seeeking proposals and will not accept unsolicited proposals. Responders are advised the Government will not pay for any information or administrative costs incurred in response to this RFI. All costs associated with responding to this RFI wil be solely at the interested party's expense. Not responding to this RFI does not prelude participation in the future RFQ, if issued. Responders are requested to provide the following information: 1. Business Name and address 2. Name of company representative and their business title 3. Business Size and Type of Small Business (if applicable) 4. Cage Code 5. UEI Number 6. Brief capabilities package demonstrating the ability to meet all the requirement in the Performance of Work Statement. 7. Provide input on PWS for industry standards, if available.
