This amendment is to update the Attachment 4 - Grease Traps SOW to the new SOW titled Attachment 4 - Grease Traps SOW 2 Aug 2021 Rev 3. ------------------------------------------------------------------------------------------------------------------------------------------ The 97th Contracting Flight, Altus Air Force Base, Oklahoma, intends to issue a Request for Quote (RFQ) for acquisition of Grease and Mud Trap Services at Altus AFB, Oklahoma. Any questions shall be submitted via email to the below points of contacts by 3 August at 1100 CST (11:00AM CST). This contract is for non-personal services to the Government. The Contractor shall provide all management, tools, supplies, equipment, transportation, personnel and labor necessary to perform pumping and cleaning services for food grease traps and mud traps at Altus AFB, Oklahoma in a manner that will ensure continuous and safe operation. See Appendix A for location of traps and capacity. Verification of field measurements and removal quantities is the responsibility of the Contractor. This requirement will begin on 01 October 2021 and will have a 12-month base period with the possibility of four 12-month option periods. This acquisition will fall under the North American Industry Classification System (NAICS) code 562998. This is a 100% Small Business Set Aside. Prospective contractors must be registered in the System for Award Management Database (SAM) prior to submission of quotes. Offerors are further advised that failure to register in the DoD SAM prior to submission of proposal will render your firm ineligible for award. Registration requires applicants to have a DUNS number. Recommend registering immediately in order to be eligible for timely award.
