Interested offerors responding to this this Combined Synopsis/Solicitation must also complete attachments 9 - DPM Bid Schedule, 10 - Past Performance Survey, and the attached SF 1449 (Blocks 12, 17, 23, 24, and any applicable items). Interested offerors must also return one signed copy of the attached SF 30 acknowledging the changes made to the original solicitation. The solicitation response date has been changed from 08 March 2019 at 11:00AM CST to 18 March 2019 at 4:30PM CST.
