Description of this Document: Sources Sought Synopsis / Request for Information (RFI) Effort Title: Survey Tool Agency/Office: 338 ESS/PKAB INTRODUCTION: This Request for Information (RFI) is issued solely for information and planning purposes. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. This RFI does not commit the Government to contract for any supply or service whatsoever. Your response to this RFI will be treated as information only and will not be used as a proposal. No entitlement to payment, of direct or indirect costs, or charges to the Government will arise as a result of a contractor's submission of a response to this announcement or the Government's use of such information. The information provided may be used by the Government in developing its acquisition strategy and statement of work/statement of objectives and performance specifications. Interested parties are responsible for marking proprietary or competition sensitive information contained within their response. Not responding to the RFI does not preclude participation in any future solicitations. The information provided in this RFI is subject to change and is not binding on the Government. PURPOSE: Air Force Personnel Center (AFPC) has a requirement to ensure all prevention activities within the Department of the Air Force (DAF) are data-informed and evaluated. Per DoDI 6400.09, AFPC must “Assess, using and established ongoing cycle, the quality and impact of primary prevention practices, policies, programs and use evaluation findings from all prevention activities to inform improvement, planning, and resourcing”. The survey tool will be utilized to evaluate training and programs within the DAF Integrated Resilience Division, to include but not limited to Total Force Annual Suicide Prevention Training, Total Force Annual Sexual Assault Prevention and Response Training, and all training for the Prevention Workforce. The Government anticipates the use of NAICS 513210, Software Publishers (size standard $41.5M), for this requirement. Survey Tool should include these capabilities: - Cloud and web-based tool accessible through mobile device - Unlimited survey creation - Custom domain and subdomain naming - Enhanced security - Interface with Power BI - Admin/Super user accounts - 24/7 customer support SUMMARY: All interested firms that possess the capabilities and capacities addressed herein are encouraged to respond to this notice by providing the information specified above no later than 4:00 PM (CST), on 18 January 2023. Information is preferred in soft-copy form in either Microsoft Word and/or Microsoft PowerPoint and shall be provided via e-mail.
