This is a combined synopsis/solicitation for the purchase of three (3) X-Ray machines. This solicitation is being issued as a(n) Request For Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2024-03, Defense Federal Regulation Public Notice 20240223, and Air Force Federal Acquisition Circular 2023-0707. In accordance with FAR 19.502-2 this requirement is a total small business set aside. The North American Industry Size Classification System (NAICS) code associated with this requirement is 334519. The Small Business Administration (SBA) SizeStandard is 600 employees. Please see the attached Salient Characteristics document detailing the specs for each machine. Additionally, please see the attached Solicitation document (FA255024Q0016). FAR provision 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. As prescribed in FAR 12.301(b)(1), the following addendum is provided for this solicitation and hereby amends any language therein. Quotes submitted in response to this solicitation shall be submitted via email no later than Thursday,16 May 2024, 1300, hours MT. In order to be considered for award, quotes shall be submitted to Mr. William Ray and SrA Nautica Bowen-Chapman at the emails listed below. 1. This is a Firm-Fixed Price (FFP) requirement. A FFP contract provides for a price that is not subject to any adjustment based on the contractor's cost experience in performing the contract. To be eligible for contract award, the vendor shall have an active System for Award Management (SAM) account. Failure to have an active SAM registration may result in the vendor being deemed ineligible for contract award. 2. Vendor shall provide the following at the time of submitting a quote: a.Commercial and Government Entity (CAGE) number b.Unique Entity Identifier (UEI) c.Point of contact information, including name, phone number, email, and company/vendor address d.Estimated delivery date/lead time e.Ensure the quote is valid for at least 30 days. 3. Questions/RFIs shall be submitted by email by email to the personnel noted below. All questions are due no later than Thursday 9 May 2024, 1300, hours MT. 4. Late quotes shall be processed in accordance with FAR 52.212(f) – Late submissions, modifications, revisions, and withdrawals of offers. No phone calls, emails only.
