The purpose of this amendment is to incorporate FAR 52.222-53 clause. Contractors utilizing this clause shall submit a statement in accordance with FAR 22.1003-4(d) that they meet the requirements as outlined and the labor categories the exemption being requested. If utilizing established catalog price, the quote shall include a catalog, price list, schedule, or other form that is regularly maintained by the contractor for the Government's review and for established market prices provide the independent sources that can be substantiated by the Government. This is a lowest price technicallly acceptable for Fall Protection services. The is for Schriever SFB, CO 80912. See attachment for details. The contractor shall provide all personnel, equipment, tools, transportation, materials, supervision, and all other items or services necessary to perform the work. The Contractor shall directly provide all necessary Fall Protection equipment to upgrade existing safety equipment. UFC 3-110-03 requires that fall protection is provided per OSHA and ANSI specs listed below. Fall protection must be installed and certified per these specs: OSHA 1910 Subpart D, OSHA 1910.28, OSHA 1910.140, and ANSI Z359. These specs have been identified in the performance work statement. RFQ date has been extended.
